Number Porting

Letter of Authorization.

Thank you for choosing BRCK as your service provider. In order to transition your current telephone number to BRCK VOIP services, BRCK must work with your previous service provider to ensure that your service is uninterrupted, and where applicable, to ensure that your number is transferred.

Numbers to port

Up to 11 rows. Use TN End only for a sequential range. Toll-free (8xx) numbers use the RESPORG process and must not be listed here. For larger ports, upload a number list below instead.

Authorization

By signing below I designate BRCK or its designated agent to transfer my service from my current provider to BRCK. By signing below I also authorize BRCK or its designated agent to transfer my current telephone number used to provide service so that BRCK may provide its service to me. By signing below, I also authorize BRCK or its designated agent to obtain billing information, customer service records and other network information required to provide me with BRCK service. I understand that I may consult with BRCK as to whether a fee will apply to the change.

Porting guidelines

All information must match the losing carrier's records exactly, or the port may be rejected.

Your 911 address can change at port — email the new service address along with this LOA.

Enter the Billing Telephone Number (BTN) once, in its own field — the master number the losing carrier has on file. Only one BTN per LOA.

List numbers in column 1 (TN Begin). For a sequential range, fill TN Begin and TN End. For non-sequential numbers, leave TN End blank and use one row each.

If no port date is requested, the default is "ASAP."

The signer must be an authorized user on record with the losing carrier.

Use separate LOAs for multiple service locations, multiple BTNs, or multiple losing carriers.

Do not enter toll-free numbers on this form — toll-free porting uses the RESPORG process.
